Practical Training Agreement ( 3 Copies )

Unemployment Fund Contribution to Practical Training Fees Student and Employer Information Form

NOTE: Identity Copy (1 piece)

Health Provision and Activation System (SPAS) Eligibility Certificate (1 piece)
